Support Coordination (Level 2)
- Explain your plan and funding to you
- Individual Support Plans
- Help you to identify and engage Service Providers
- Link you to services in the community
- Assist you when you are working with your providers
- Explore housing and employment options
- Assist you to monitor the budget of your plan
- Assist you in obtaining or completing reports for the NDIS
- Hospital Discharge Planning
- Transitioning between services such as Home, Hospital, Supported Accommodation, and residential care
- Report writing
Specialist Support Coordination (Level 3)
- A higher level of support for participants whose situations are more complex and who need specialist support
- Assist manage challenges in complex service delivery environments

Who Can Access Support Coordination?
Support Coordination is funded through the National Disability Insurance Scheme (NDIS) and may be included in your plan to help you implement and manage your supports effectively.
If you believe you would benefit from this service, speak with your NDIS Planner or Local Area Coordinator (LAC) during your planning meeting. They can help determine whether Support Coordination is suitable for your needs and include it in your plan accordingly.
